Black Hole Sign: Characteristic Feature of Fat Necrosis on Breast MRI

Abstract
Purpose: The purpose of this study was to describe the findings of fat necrosis on breast MRI.

Materials and Methods: Between January 2007 and December 2011, histologically confirmed 10 lesions of fat necrosis of breast in 9 women (median age, 52 years; range 32-60 years) were included. Two radiologists retrospectively analyzed the findings of breast MRI in consensus. Morphology was analyzed according to the Breast Imaging Reporting and Data System. The median signal intensity of central portion of the enhancing lesion was compared with that of subcutaneous fat layer on T1-weighted MR images.

Results: Median lesion size was 2.1 cm (range, 0.6-5.5 cm). Ninety % (9/10) of the lesions were a mass types and the remaining 10% (1/10) showed non-mass enhancements. Eighty nine % (8/9) of the masses showed an irregular margin. All mass lesions (9/9) showed an evenly thin, rim-enhancement. The median value of signal intensity of the central component of the lesions was lower than that of subcutaneous fat (51 vs. 167, P<0.05, Wilcoxon signed rank test) for the lesions more than 1.0 cm. The majority of the lesions showed persistent (80%, 8/10) or plateau (20%, 2/10) pattern kinetic curve pattern.

Conclusion: Characteristic finding of the fat necrosis on breast MRI is an irregular, rim-enhancing mass with markedly lower internal signal intensity than that of subcutaneous fat.
